Welcome to the Health Mentors Program Faculty Toolkit!
Our toolkit is developed by an interprofessional group of faculty from Jefferson Medical College, Jefferson Schools of Health Professions, Nursing and Pharmacy. We've developed this toolkit as a self-directed learning and educational resource for faculty and educators with an interest in interprofessional education. The toolkit is based on principles of interprofessional collaborative practice competencies, team-based learning, and pedagogical teaching strategies.
The toolkit provides the following information:
- Overview of the Health Mentors Program
- World Health Organization (WHO) International Classification of Functioning, Disability and Health (ICF) (PPT | PDF)
- How to facilitate an interprofessional small group? checklist / tip sheet - coming soon
- Interprofessional Small Groups 'Frequently Asked Questions'
Acknowledgements
