Q: What if MP had mild New York Heart Association (NYHA) class I or II CHF? Would a TZD be contraindicated?

A: In clinical trials, both currently available TZDs (rosiglitazone and pioglitazone) included some patients with NYHA Class I or II CHF status, but not those with Class III or IV CHF. TZDs can be used cautiously at lowest doses (eg, rosiglitazone 2 mg/d; pioglitazone 15 mg/d) with close observation and gradual dose escalation.
