Q: Metformin did not work before in this patient, and her predominant physiologic defect is beta cell dysfunction. Why would you consider metformin, which primarily improves insulin utilization in the liver, now?

A: Metformin does not work effectively unless sufficient insulin is available in the body.8 The initial trial with metformin may have failed, because PD did not have sufficient insulin secretion. With insulin now delivered exogenously, drugs that improve the effectiveness of insulin action may provide an additional benefit in overall glycemic control.
