KM is a 43-year-old woman, who was originally born in India but has lived in the US since she was 19 years old.

Remembering to recommend all appropriate screening tests and lifestyle changes demands a systematic approach to preventive care. Paper-based or computerized reminders based on the patient demographic characteristics are critical tools that have been proven to improve preventive care delivery.

Bringing patients back to complete a preventive care visit is a useful strategy to ensure that a patient has followed through on recommended tests.

Patient Status

KM has done very well. She has been consistent about exercising. Her eating habits were always fairly good but she has modified food choices and portion size a bit more. She has tolerated metformin at 500 mg BID. Her A1c has come down to 6.1% and she has been able to maintain this value for over 2 years. Blood pressure medication has not been necessary, but statin therapy was initiated to reach her LDL goal.
