Forms and Procedures to Reproduce or Publish Materials

1. Patrons must read and agree to the policies and procedures outlined in the Rights and Reproduction Policy.
   
   
2. Patrons must complete a "Reproduction Request Form" to order reproductions and/or request permission to publish materials from the collections of the University Archives/Special Collections Department.

3. If Archives staff approves the request to publish, the patron will be required to sign the "Publication Rights Agreement." This completed form constitutes permission for the patron to reproduce the specified image(s) in the single publication or production described in the "Reproduction Request Form."

4. When all fees are paid, an authorized member of the Archives staff will sign the "Publication Rights Agreement." A copy of the completed agreement will be enclosed with the images when they are sent to the patron.
