State Resources
- National Academy for State Health Policy (NASHP)
A non-profit, non-partisan organization dedicated to helping states improve health policy and practice. Its Patient Safety Toolbox for States lists state-based reporting systems on policies, practices, forms, reports, methods, and contracts, and reports, fast facts and issues related to patient safety.
- Pennsylvania Patient Safety Authority
State agency that receives reports on "serious events" and "incidents" from hospitals and ambulatory surgical facilities. It identifies problems and recommends solutions that promote patient safety. Free, quarterly "Patient Safety Advisory" contains articles with scientific rigor as well as practical strategies and toolkits for educational programs.

From the Agency for Healthcare Research and Quality, a portal to a wealth of resources on all aspects of quality and safety.
- AHRQ QualityTools
A clearinghouse for practical, ready-to-use tools for measuring and improving the quality of health care. Organized into 4 categories: Providers, Policymakers, Patients & Consumers, and Payers and Purchasers
- AHRQ National Quality Measures Clearinghouse™ (NQMC)
A public repository for evidence-based quality measures and measure sets, updated weekly. Organized by measure name, developer, disease, treatment, organization, and most viewed measures.
- AHRQ Web M & M: Morbidity & Mortality Rounds on the Web
Free to all users from 2003 to present. Choose the “case archives” or “perspectives on safety.”
- Patient Safety Network (PSNet)
Weekly updates of patient safety literature, news, tools, grants and meetings ("What’s New"), and a vast set of carefully annotated links to important research and other information on patient safety ("The Collection"). Browsable by resource type or subject.
- CDC on Hand Hygiene
The Centers for Disease Control & Prevention (CDC) provides guidelines, slides, posters and other materials on promoting hand hygiene in healthcare settings.
- ECRI Patient Safety Center
ECRI, a nonprofit institute, evaluates various procedures, devices & drugs for safety, efficacy and best practices. Their opinions are well-regarded and used to help improve health care quality. This page provides public resources on patient safety. Membership with ECRI also provides access to the member-only website with additional resources. Other services/consulting is also available to members.
- Institute for Healthcare Improvement
Not-for-profit organization spearheading the "5 Million Lives Campaign," an outcomes-based initiative recommended to Pennsylvania healthcare facilities by the Patient Safety Authority. Material is organized by topics, including Patient Safety.
- Institute of Medicine of the National Academies HealthCare & Quality Topics
Links to reports and projects from the Institute of Medicine in its role providing independent, evidence-based advice to improve the nation's health, including an online version of "To Err Is Human," which brought the issue of medical errors to national attention.
- Joint Commission: Facts about Patient Safety (formerly JCAHO, Joint Commission on Accreditation of Healthcare Organizations)
A major repository of patient safety resources and information relevant for patients, physicians, nurses, pharmacists, health care institutions and allied professionals. Professional resources include standards such as the “Do Not Use” List; Hospitals, Language, and Culture; Infection Control; National Patient Safety Goals; Patient Safety Practices; Solutions; Speak Up; Universal Protocol. Quality Check includes each accredited organization’s most recent Quality Report.
- Joint Commission International Patient Safety Resources
A major repository of resources and information related to all aspects of patient safety that will be relevant for patients, families, physicians, nurses, pharmacists – as well as for all health care institutions and allied professionals.
- Leapfrog Group - For Hospitals
Formed by a group of businesses that buy healthcare for employees, the Leapfrog project aims to reduce costs by preventing medical mistakes and improving quality of care. The Hospitals section includes free webinars, case studies and information about the Leapfrog Hospital Quality and Safety Survey and Leapfrog Hospital Rewards Program.
- National Quality Forum
A private, not-for-profit membership organization created to develop and implement a national strategy for healthcare quality measurement and reporting.
- Partnering to Heal (HHS)
Interactive training videos for healthcare students and professionals about the prevention of healthcare-associated infections, produced by the U.S. Department of Health and Human Services.
- Patient Safety Education Project (PSEP)
A curriculum driven, train-the-trainer approach to disseminating patient safety practices nationally and internationally.
- VA National Center for Patient Safety
Presentations, toolkits, advisories, newsletters and other publications aimed at developing a culture of safety throughout the Veterans Health Administration, but applicable to other healthcare organizations.
